  • excision of tumour
  • cinacalcet
    • decreases PTH secretion

• increased [PTH] due to decreased feedback resulting from chronic hypocalcaemia in chronic renal failure;
  • (as decr. 1alpha-hydroxylase ⇒ decr. 1,25)
  • ⇒ hyperphosphataemia which ⇒ further increased Ca excretion & further hypocalcaemia;
• can be Rx with calcitriol (the active form of vitamin D) but this risks hypercalcaemia and hyperphosphataemia
  • new drug: paricalcitol which is a vitamin D analogue with less risk of hypercalcaemia
