Introduction
Primary hyperparathyroidism due to functional tumours causes
- hypercalcaemia, hypophosphataemia, osteitis cystica, Ca stones kidneys;
- Rx options include:
- excision of tumour
- cinacalcet
- decreases PTH secretion
Secondary hyperparathyroidism
- increased [PTH] due to decreased feedback resulting from chronic hypocalcaemia in chronic renal failure;
- (as decr. 1alpha-hydroxylase ⇒ decr. 1,25)
- ⇒ hyperphosphataemia which ⇒ further increased Ca excretion & further hypocalcaemia;
- can be Rx with calcitriol (the active form of vitamin D) but this risks hypercalcaemia and hyperphosphataemia
- new drug: paricalcitol which is a vitamin D analogue with less risk of hypercalcaemia
