Western Health Hospital in the Home service (HITH)

WH intranet policies

HITH referral process from ED

all referrals

  • Patient reviewed by Senior Emergency Department Medical Officer and patient assessed as suitable for HITH
  • ED Consultant/Registrar to contact appropriate treating unit to propose admission of patient under their unit bed card
  • Medication Chart for HITH & Discharge Script to be completed by medical team for patient. Ensure that

one dose of the medication has been given to the patient before transfer to HITH

  • The new process aims for continuity of care by ensuring the patient is managed and reviewed by only one inpatient unit, and not just the registrar on duty for the day as was the case previously. This inpatient unit will be the one initially referred.

business hours

  • refer patient to inpatient treating unit of the day (usually the registrar for general medicine)
  • treating unit accepts patient under HITH
  • referral to HITH Liaison Nurse made via iPM including reason for transfer to HITH
  • contact Liaison Nurse on ext. 51793
  • Liaison Nurse and inpatient unit will review patient in ED prior to discharge or arrange a process as per After Hours process.

after hours

  • refer patient to inpatient treating unit of the day (usually the registrar for general medicine)
  • treating unit accepts patient under HITH and informs ED of the time the next day when that treating unit will review the patient with HITH
    • for inpatients in ED SSU, this will usually be in SSU
    • for patients discharged home overnight for review Mon-Fri, this will be as follows:
      • Western Hospital patients: Footscray 4 West ward
      • Sunshine Hospital patients: Sunshine 1st floor cardiology room
    • for patients discharged home overnight for review Saturday or Sunday, this will be as follows:
      • review in the Emergency Dept but ED staff next morning must call ext 50349 to speak to Liaison Nurse that morning of the review to pass on the patient details and time patient will be returning for review by inpatient unit
  • referral to HITH Liaison Nurse made via iPM including reason for transfer to HITH AND the time and place for review the next day

initial hospital review by Liaison Nurse and inpatient unit

  • Treating Unit Registrar will clearly document a Management Plan to include arrangements for ongoing review of patient
  • HITH Liaison Nurse will be present in the ED Mon-Fri morning to assess referred patients in the SSU / ED and will collect

paperwork and the medication chart for referrals made after hours. Sat/Sun: Liaison Nurse will collect at the review.

  • Liaison Nurse will assess the patients and organise supplies, visit times and reviews for the patients prior to sending them home
