CHADS2 score for determining stroke risk in patients with non-valvular AF
scoring system
- score 1 point for each item present, but 2 points for PH stroke or TIA:
| PH congestive cardiac failure |
| PH hypertension |
| age > 74 yrs |
| diabetes mellitus |
| PH stroke (CVA) or transient ischaemic attack (TIA) |
risk of stroke in patients with AF
| score | annual risk of stroke if no anticoagulation |
|---|---|
| 0 | 1.9% |
| 1 | 2.8% |
| 2 | 4.0% |
| 3 | 5.9% |
| 4 | 8.5% |
| 5 | 12.5% |
| 6 | 18.5% |
- most advise commencing anticoagulation in AF patients if CHADS2 score is 2 or more and there are no significant C/I to warfarin - see antithrombotic Rx for prevention of stroke
- HOWEVER, there is a substantial move AWAY from using CHADS2 score to the newer CHA2DS2-VASc score for determining stroke risk in patients with non-valvular AF as the CHADS2 score of 0 does not accurately assign risk given that significant risk occurs in those aged > 64yrs, and in women.
