ovarian hyperstimulation syndrome (OHSS)

Introduction
- a complication of IVF therapies which although most cases are mild, can result in severe illness and generally lasts 1-2 weeks
Clinical features
mild OHSS
- ovarian enlargement to 5-12cm
- ascites with mild abdominal distension
- abdominal pain
- nausea
- diarrhoea
- mildly raised D-Dimer pathology test
severe OHSS
- additional features may include:
- ascites
- hematocrit > 45%
- WBC > 15,000
- oliguria
- creatinine 1.0-1.5 mg/dl
- creatinine clearance > 50 ml/min
- liver dysfunction
- anasarca.
critical OHSS
- may include:
- enlarged ovary
- tense ascites with hydrothorax and pericardial effusion
- hematocrit > 55%
- WBC > 25,000
- oligoanuria,
- creatinine > 1.6 mg/dl
- creatinine clearance < 50 ml/min
- kidney failure
- thromboembolic phenomena
- ARDS
Complications
- may be complicated by:
- ovarian torsion
- ovarian rupture
- venous thromboembolism
- acute respiratory distress syndrome
- electrolytes imbalance
- thrombophlebitis
- chronic kidney disease
